you may want to take a guided tour of the Roscommon facility that
most closely matches your needs.

- Make an appointment for the family and/or patient to take a

If you or your loved one choose a Roscommon facility, you will encounter
an admissions process that has been streamlined to be as quick and
easy as possible.
Each new resident undergoes an assessment and case review to ensure
that Roscommon will best meet his/her clinical needs. If the patient
is hospital-bound, our clinical liaison will visit the patient and
perform the assessment at the facility.
Our staff has a thorough understanding of the intricacies of health
insurance and third-party providers. We will work closely with you
to ensure the maximum utilization of these benefits on your behalf.
When the admissions date is set, Roscommon is at your side again to
smooth the transition from home or hospital. Our staff social worker
is experienced in the many issues that can accompany this decision,
and is available to patient and family members as needed.
When a patient is admitted, our interdisciplinary team begins a close
relationship with the resident, family members, and the personal physician
to assess, develop, and coordinate a comprehensive care plan. Our
medical director, nursing supervisors, and therapists evaluate each
resident's needs, goals, and personal interests to ensure the most
beneficial therapeutic experience possible. As the resident's progress
and goals are achieved, the care plan is evaluated for effectiveness,
and is adjusted accordingly. |
